2025 Research Report
I am thrilled to share with you the 2025 Research report for the Department of Radiation Oncology, Princess Alexandra Hospital.
During the past year we have offered 14 trials to suitable participants, with an additional 23 studies closed to accrual but continuing in follow-up phases. It is important to acknowledge the 1021 people who have graciously agreed to participate on these studies, and the hard work of our staff from the Cancer Trials Unit who have made this possible. Going forwards, access to new clinical trials has also been bolstered through our recent Affiliation to NRG Oncology a leading cancer cooperative trials group in North America, enabling us to bring new and innovative trials to patients within Metro South.
This year, investigators from our department have been awarded AU$ 2.75 M in research grant funding including NHMRC funding to support am internationale phase 3 trial of high precision radiotherapy treatment in prostate cancer. This is a significant investment and highlights the capability, impact and promise that our work brings across Australia and beyond. I would also like to acknowledge and thank generous funding support from Metro South Health Research Support Scheme and PA Research Foundation. Through these local competitive grant opportunities, we can encourage early-career and new research capacity within our craft groups.
We recorded 41 peer-reviewed publications across a vast range of topics; radiotherapy technical innovation, artificial intelligence, head and neck cancer, brain cancer and supportive care research were the most common themes. Two of our highest-impact papers included international consensus guidelines for head and neck cancer published in Lancet Oncology and results from the De-squamate Study in skin cancer, published in Journal of Clinical Oncology. Congratulations to A/Prof Lachlan McDowell and Dr Howard Liu (Radiation Oncologists) who were co-authors on these important publications.
There were 41 conference abstracts presented by staff from our department across a range of national forums representing Medical, Radiation Therapy, Medical Physics and Cancer Collaborative groups. There were also international presentations delivered in USA and UK.
Currently there are 13 team members working towards higher degrees (8 PhD and 5 Masters) within Medical Physics, Nursing, Clinical Oncology and Radiation Therapy. This commitment to progress our field and ultimately improve care for patients is commendable, as is the support from staff acting as supervisors.
Finally, I would like to recognise G Wright and B Mander (QUT students), supported and supervised by A/Prof Cathy Hargrave (Principal Research Fellow), who were awarded Best Radiation Therapist Student Paper Prizes at National Conferences.
It is encouraging to reflect upon these significant contributions and achievements. All within Radiation Oncology should be proud of the continued person-centred research, technological innovation and quality service development we have been able to deliver over the past 12 months. I am grateful to Rochelle Palleson, Sharon Watson, Anna Koch, Nakia Beaton, A/Prof Elizabeth Brown, A/Prof Cathy Hargrave and Andrew Puffett who have helped to carefully curate this information and present it so professionally.

In 2025, the Radiation Oncology Trials Team prioritised the completion of legacy studies, dissemination of research outcomes through high-impact publications, and the strengthening of international collaborations to support future clinical trials. A key strategic focus was the development of partnerships with leading international research groups, including NRG Oncology, to enable participation in global collaborative studies and expand trial opportunities for patients.
Several long-standing radiation oncology trials reached completion during the year, resulting in multiple peer-reviewed publications. Two studies of particular significance were the FASTRACK II trial and the SUPREMO study.
The TROG 15.03/ANZUP 16.001 FASTRACK II trial compared stereotactic ablative body radiotherapy (SABR) compared with radiofrequency ablation and cryoablation for the treatment of primary renal cell carcinoma. A cost-effectiveness analysis demonstrated that, over a ten-year period, SABR was the dominant strategy in the Australian setting, providing gains in quality-adjusted life years while reducing per-patient costs for patients ineligible for surgery. These findings, published in The International Journal of Radiation Oncology, Biology and Physics, support SABR as the preferred treatment option for this population. The MRC SUPREMO trial (BIG 2-04) investigated the selective use of postoperative radiotherapy following mastectomy in breast cancer. Ten-year survival outcomes for post-mastectomy chest wall irradiation were published in The New England Journal of Medicine with the ROPART site contributing to this international collaboration. Results demonstrated no overall survival difference between patients who received chest wall irradiation and those who did not. (DOI: 10.1056/NEJMoa2412225). It is recognised that patients in this trial were treated in an earlier era, with systemic therapies and multidisciplinary care models differing from contemporary practice. As treatment decisions increasingly incorporate biological stratification and genomic profiling, ongoing research in this area remains essential.
In parallel, the department advanced several new clinical trials. AXIOM, a phase II multicentre randomised trial evaluating stereotactic ablative body radiotherapy combined with immunotherapy versus immunotherapy alone in treatment-naïve patients with oligometastatic extracranial melanoma, opened to recruitment. EvoPAR Prostate 02, a phase III randomised, double-blind, placebo-controlled trial of adjuvant Saruparib in patients with BRCA-mutated, localised high-risk prostate cancer receiving radiotherapy and androgen deprivation therapy, achieved site activation. ROSALIE, a study investigating radiation omission in clinically node-negative breast cancer patients undergoing lumpectomy with pathological complete response following neoadjuvant chemotherapy, entered the start-up phase. Notably, the department’s first NRG Oncology trial, NRG-HN014, received ethical approval, with international regulatory processes underway in preparation for recruitment.
The achievements of 2025 reflect the sustained commitment and collaborative efforts of the multidisciplinary Cancer Trials Team. Their expertise and engagement remain central to the delivery of high-quality clinical research and to improving outcomes for patients with cancer.
